For Control numbers 12, 13, and 24 (FX Type 1, 2, and 3), is there a menu listing the effects assigned thru 1-127?
i.e. 1-10 = Reverb, 11-20 = BMP Delay, etc.

Yes, this information is available in the emx/esx midi implementation guides. It is in hex so to get the decimal values you'll need good ol' windows calculator in scientific mode to convert back and fourth.
