My PA 588 halts at Loading Package...

Help!
Since my Pa588 has some strange behavior recently, I decide to do a system restore.

But after I insert the SD card with the pkg file and boot while pressing Intro 1 button, the screen shows up "Loading Package..." then never goes on the next step. Next I have tried both NAND clear and format, PA 588 still halts at Loading Package...

I have searched forum for similar problem. I will try other SD cards, but I can't format Sd card in PA588 any more.

BTW, I have noticed in a thread that there's a method to restore OS in PA 588's USB mode, but ther's no further infomation. Any one can help with this?
